Job summary

Early Break in Rochdale is seeking a Young People’s Specialist Substance Use Worker to provide 1:1, face-to-face support to young clients who may be at risk due to substance use or other vulnerabilities. Evening and weekend sessions may be required.

You will assess needs, develop SMART care plans, and deliver structured interventions, including substitute prescribing where appropriate, while liaising with families, agencies, and communities to ensure effective care.

Qualifications

• Experience delivering 1:1, face-to-face work with young people.
• Ability to develop SMART care plans and monitor progress.
• Knowledge of safeguarding and working within policies.
• Willingness to work evenings and weekends.

Responsibilities

• Deliver 1:1, face-to-face work with young clients and their parents/carers.
• Complete a full assessment of each client’s needs and develop SMART care plans.
• Offer structured treatment interventions, including substitute prescribing if required.
• Work in community settings and liaise with partner agencies to meet treatment needs.
• Offer drop-ins in relevant young people’s settings.
• Contribute to multi-agency meetings as key worker/advocate.
• Support clients into inpatient treatment and arrange after-care where needed.
• Adhere to safeguarding policies and data protection requirements.
• Maintain knowledge through ongoing training and contribute to charitable activities.
