Job summary

Early Break in Rochdale is seeking a Young People’s Specialist Substance Use Worker to deliver 1:1 support to young people and families. You will assess needs, develop SMART care plans, and provide structured interventions, including substitute prescribing where appropriate. The role involves community work and multi-agency collaboration.

The post is full-time (37 hours) with safeguarding duties and data protection responsibilities, offering strong staff benefits.

Qualifications

Experience in delivering direct support to young people.
Ability to assess needs and develop SMART care plans.
Ability to deliver structured interventions for substance use.

Responsibilities

Deliver 1:1 face-to-face work with young clients and parents/carers, including evenings/weekends if needed.
Complete assessments and monitor SMART care plans.
Provide structured treatment interventions, including substitute prescribing if appropriate.
Engage with community settings to reach clients.
Liaise with agencies to meet treatment needs and attend multi-agency meetings.
Lead as needed in safeguarding and record keeping with DP compliance.
