Job summary

Early Break is seeking a Young People’s Specialist Substance Use Worker to deliver 1:1, face-to-face support to young people and their families. You will assess needs, develop SMART care plans, and provide tailored interventions within community settings, with some evening and weekend sessions.

You will liaise with partners, contribute to safeguarding and after-care planning, and maintain accurate records using our database. Closing date: 5 November 2026.

Qualifications

• 1:1 face-to-face work with young people and parents/carers.
• Complete SMART care plans and regular reviews.
• Ability to work evenings and weekends.
• Understanding of safeguarding policies and procedures.
• Record and report client information using the central database.

Responsibilities

• Deliver 1:1, face-to-face work with young clients and their parents/carers; manage appointments.
• Complete a full assessment of needs and develop SMART care plans.
• Offer structured treatment interventions, including substitute prescribing when appropriate.
• Engage with community settings to reach potential clients.
• Liaise with partner agencies and participate in multi-agency meetings.
• Support after-care and transition back to the community as needed.
• Adhere to safeguarding policies and lead when required.
• Maintain accurate client records in the database.
