With You is seeking a dedicated Young Persons Worker to support a caseload of young people facing substance misuse challenges. You will assess needs, create care plans, deliver treatments and refer to specialists as required, while developing plans to help clients regain control of their lives.

You will engage proactively to provide support, education and guidance, building meaningful relationships. A driving licence and access to a vehicle are essential, with a home-based role and outreach in

Formación

• OCN/NVQ level 3 or equivalent
• Experience managing a caseload of young people with complex needs
• Sound knowledge of safeguarding and applying safeguarding processes
• Evidence of multi-agency working
• Evidence of continuing professional development
• Excellent relationship-building skills across diverse backgrounds
• Ability to work both independently and as part of a team, sharing skills and knowledge
• Strong organisational skills and time management under pressure
• Ability to record and maintain accurate case details promptly
• Adequate IT skills across different packages and digital media
• Access to a vehicle and a full UK driving licence

Responsabilidades

• Manage a caseload of young people with difficulties, assess needs and develop care plans
• Deliver a range of treatments and refer to specialist support when needed
• Engage with young people to provide support, education and guidance
• Build strong relationships with clients and key stakeholders
• Work as part of a team to ensure a high-quality service and effective collaboration
• Record and maintain accurate case details and progress notes
• Utilise IT tools and digital media to document work and communicate with partners
