Job summary

Your Housing Group is recruiting a Project Officer/Support Worker for the Bridge Foyer in Chester. You will provide key worker support to 31 young residents aged 16-25, guiding them through bespoke action plans to build independent living skills and resilience.

The role involves a 3-week rolling rota, some lone working and close multi-agency collaboration. You will bring empathy, flexibility and a proactive approach to helping young people move toward their goals, with a supportive team and

Qualifications

• Provide key worker support to young people through bespoke action plans.
• Help residents develop independent living and life skills.
• Build relationships with residents, colleagues and external agencies.
• Work on a 3-week rolling rota and occasional lone working.
• Safeguarding and welfare checks as part of the role.

Responsibilities

• Deliver 1-2-1 support sessions on life skills.
• Review action plans and progress with residents.
• Support residents to attend medical appointments.
• Contribute to multi-agency meetings and group activities.
• Manage own diary and caseload.
