Job summary

Oxfordshire County Council is seeking a Leaving Care Personal Advisor to support care-experienced young people. You will build trusted relationships, help them create pathways to independence, and coordinate services across housing, health and education to improve outcomes.

You will manage a caseload, advocate for young people, and work with interpreters and partner agencies to navigate complex systems. This role emphasizes empowering young people and promoting their voice in decision-making.

Qualifications

• Experience as a case manager or working with care-experienced young people.
• Ability to coordinate services and work with partners.
• Experience with UASC processes and immigration status awareness.
• Strong communication and advocacy skills.
• Willingness to work with interpreters.

Responsibilities

• Act as a case manager and build positive relationships with young people.
• Empower young people to participate in decisions, involving peer consultation when appropriate.
• Assess needs and coordinate/participate in needs assessments.
• Develop and review pathway plans to meet needs, including triple track planning.
• Provide information, advice, planning and holistic support across health, housing and post-16 careers.
• Work with interpreters and raise issues when necessary.
• Support with the UASC process and follow guidelines for immigration status.
• Ensure young people are aware of and access financial entitlements.
• Liaise with housing, EET, vulnerable adults and health services.
• Coordinate services and ensure uptake by the young person.
• Monitor progress and well-being with supporting agencies.
• Participate in a duty system and initial welfare checks for spontaneous asylum seekers who are children arriving in Oxfordshire.
