Job summary

Lincolnshire Partnership NHS Foundation Trust (LPFT NHS) is recruiting for a Keyworker role to support young people, families and carers in Boston and surrounding areas. The post focuses on non-clinical, trauma-informed assistance to navigate health and social care systems and reduce admission risk.

You will manage caseloads, coordinate multi-agency care, and empower families to engage with complex services while safeguarding and promoting independence.

Qualifications

• Must have lived or learnt experience of autism and/or learning disabilities.
• Experience in non-clinical support work with children and families is desirable.

Responsibilities

• Manage a diverse caseload of children, young people and families, delivering flexible, person-centred and trauma-informed support.
• Act as a consistent point of contact, coordinating care across health, education, social care and the voluntary sector.
• Support families to navigate complex systems, providing clear information and enabling access to the right support.
• Build trusted relationships and ensure the voice of the young person and family is heard in planning and decision-making.
• Identify, assess and respond to risk and safeguarding concerns in line with policy.
• Work to prevent hospital admissions and support safe discharge and transitions, including into adult services.
• Signpost and refer to appropriate services and community resources, promoting independence and community-based support.
• Monitor progress against agreed outcomes, contributing to personalised care planning and step-down arrangements.
• Maintain accurate, timely records and contribute to service evaluation and improvement.
• Support and guide a Keyworker Support Worker, ensuring delegated tasks are completed safely and effectively.
