Job summary

Wirral Council is recruiting a Keyworker for the WALK team to support 0–25-year-olds with autism and/or learning disabilities. The role involves holistic work with young people and families, coordinating with professionals to reduce risks and improve outcomes.

Responsibilities include managing caseloads, supporting multi-agency collaboration, and organising Community Treatment Reviews when needed. DBS check and UK right-to-work are required; training on local systems will be provided.

Qualifications

• Experience of mental health support with children and/or adults is desirable.
• Confidence in using digital platforms such as Microsoft Office and recording systems (e.g. Liquid Logic) is desirable; training on Local Authority systems will be provided.

Responsibilities

• Work holistically with young people (0–25) with autism and/or learning disabilities and their families, as well as professionals across the system.
• Manage a caseload supporting other professionals to collaborate and undertake agreed actions in a timely fashion, with support from fellow keyworkers and the WALK Team Leader.
• Help organise and support Community (Education) Treatment Reviews when necessary for their cases.
• Support individuals transitioning from children to adult services, ensuring needs are met within the local community and during/after the transition.
