Autism & LD Keyworker: Family & Multi-Agency Support

Zusammenfassung

Wirral Council is recruiting a Keyworker for the WALK team to support children and young people (0-25) with autism and/or learning disabilities and their families. You will work holistically with professionals across the system, organise actions, and help coordinate multi-agency reviews to meet needs.

Hybrid working and a fixed-term 12-month contract with NHS England funding, offering a supportive environment and opportunities to develop in the Family Help service.

Qualifikationen

• Experience or understanding of mental health support for children and/or adults is desirable.
• Ability to work with young people with autism/learning disabilities and families.
• Strong multi-agency collaboration and communication skills.

Aufgaben

• Manage a caseload and support collaboration between professionals.
• Organise and support Community (Education) Treatment Reviews.
• Assist in coordinating multi-agency plans to address risks and needs.
