Job summary

North London NHS Foundation Trust is recruiting an LD and Autism Keyworker to support children and young people with learning disabilities and/or autism and behaviours of concern across North Central London.

You will provide high-quality, coordinated support to families and carers, bridging health, education and social care, while working as part of a multidisciplinary team to reduce crisis interventions and avoid unnecessary Tier 4 admissions.

Qualifications

• Provide high-quality support to children and young people with learning disabilities and/or autism and behaviours of concern.
• Coordinate care across health, education and social care.
• Engage families/carers as key points of contact in the LD & Autism Programme.

Responsibilities

• Provide high-quality support to children and young people and their families/carers as part of a multidisciplinary team.
• Coordinate care across health, education and social care to unblock systems and bridge services.
• Work with Tier 4 providers, CAMHS, social care and education to ensure joined-up support for the young person.
• Empower families to navigate barriers and access the right support from the system.
