Job summary

Wirral Council is recruiting a Keyworker for the WALK team to support 0–25-year-olds with autism and/or learning disabilities. The role is NHS England funded and involves holistic work with young people and families, coordinating across professionals to address needs.

You will manage a caseload within a multi-disciplinary team, organise Community Treatment Reviews when needed, and work to reduce risk and improve outcomes. DBS check and right to work in the UK are required.

Qualifications

• Experience supporting individuals transitioning from children to adult services.
• Experience in mental health support for young people with autism or learning disabilities.
• DBS check and right to work in the UK requirements.

Responsibilities

• Work holistically with young people aged 0–25 with autism/LD and their families.
• Coordinate with professionals across education, health and social care to address needs.
• Manage a caseload and support collaboration with other professionals to undertake agreed actions.
• Organise and support Community Treatment Reviews when necessary for cases.
