Young People Leaving Care Outreach Support Worker

Overview

This is a long-term temporary position with no specified end date, ideal for quick starts through a temp-to-permanent pathway. Join a well-established company renowned for supporting young people, offering high-quality provisions and a market-leading approach in this sector.

Working Hours

Hours are between 8am and 8pm, Monday to Friday. Flexibility is required to meet the needs of the young people, and the position offers a degree of flexibility for the worker.

Key Responsibilities
  1. Support young people in developing independence skills and assist with property inspections.
  2. Manage diaries and work closely with permanent staff.
  3. Provide transport and escort services as per care plans.
  4. Conduct welcome and induction sessions for new young people.
  5. Perform ongoing assessments of service users’ needs.
  6. Support and monitor service users, including welfare and NHS issues.
  7. Prepare reports and arrange appointments with partner agencies.
  8. Maintain regular contact with service users and inform them of support groups.
  9. Assist in the move-on process for service users.
  10. Record, update, and maintain electronic records accurately.
  11. Attend meetings and provide mediation as required.
  12. Utilize IT packages effectively and work cooperatively within multidisciplinary teams.
  13. Maintain confidentiality and assist in policy development.
  14. Report housing issues and participate in on-call duties and reception duties as needed.
Qualifications & Requirements
  • Experience supporting young people and their families.
  • Full UK driving license with business insurance is essential.
  • Current online DBS for a quick start.
  • Minimum of NVQ Level 3 Children & Young People Workforce or equivalent.
  • High standard of general education.
  • Willingness to undertake further training.
Job Purpose

To provide support for vulnerable young people in line with care plans, working closely with social workers and other agencies to promote independence and well-being.
