Job summary

21 Health and Social Care seeks a Wraparound Support Worker to help children, young people and families in community settings. You will build trust, promote wellbeing, and support families to navigate challenges with a person-centred approach.

Qualifications

• Experience supporting children, young people, or families in care/community settings.
• Strong communication and relationship-building skills.
• Ability to work flexibly including evenings or weekends.
• Trauma-informed, child-centred approach.

Responsibilities

• Provide 1:1 and family-based support to children and young people.
• Deliver practical, emotional, and behavioural support in home and community settings.
• Work with families to identify strengths and set achievable goals.
• Support routines, boundaries, education engagement, and emotional regulation.
• Liaise with social work, education, health, and third-sector services.
• Maintain accurate records and contribute to care plans.
• Promote safeguarding, wellbeing, and positive outcomes.
