Job summary

21 Health and Social Care are recruiting a Wraparound Support Worker to help children, young people and families in community settings. You will provide tailored 1:1 and family-based support, build trusted relationships and promote wellbeing while supporting routines and education engagement.

The role requires compassionate, reliable individuals with experience in care, strong communication, and a trauma-informed approach.

Qualifications

• Experience with children/young people in care or community settings.
• Ability to work flexible hours including evenings/weekends.
• Trauma-informed, child-centred approach.
• Strong communication and relationship-building abilities.

Responsibilities

• Provide 1:1 and family-based support to children and young people.
• Deliver practical, emotional, and behavioural support in home and community settings.
• Work with families to identify strengths and set goals.
• Support routines, boundaries, education engagement, and emotional regulation.
• Liaise with social work, health, education, and third-sector services.
• Maintain accurate records and contribute to care plans.
• Promote safeguarding, wellbeing, and positive outcomes at all times.
