Job summary

21 Health and Social Care are recruiting a Wraparound Support Worker to work with children, young people and families in the community across Scotland. You will provide tailored, 1:1 and family-based support, build trusting relationships, support wellbeing and help families navigate challenges in home and community settings.

Flexible working options include evenings and weekends as required, and you will liaise with other professionals to coordinate care.

Qualifications

• Experience supporting children, young people or families in care or community settings.
• Strong communication and relationship-building skills.
• Ability to work evenings or weekends if required.
• Trauma-informed, child-centred approach.

Responsibilities

• Provide 1:1 and family-based support to children and families in community settings.
• Deliver practical and emotional support and help with wellbeing.
• Collaborate with families to identify goals and connect with services.
• Maintain records and contribute to care plans.
