Job summary

Mind in Bradford is seeking a dedicated SMI-LE support worker to provide one-to-one mental health support. The role involves signposting, collaboration with other services, and travel across Bradford district to meet service members and offer personalised assistance.

The role requires a good grasp of local support services, safeguarding, and data protection. Flexible working options, including hybrid/remote where suitable, are available.

Role Background

The SMI-LE service involves providing one-to-one support for adults experiencing moderate to severe mental health challenges, focusing on areas affecting well‑being, including employment, welfare and benefits, social housing, money management, social isolation, and bereavement. A good understanding of local support services across the district is essential, as the role involves signposting service members to relevant services for additional support. Support is flexible and may take place in the service member's home, in the community, or at the local Community Mental Health Team base. It can be delivered face‑to‑face, by telephone, or by video call, depending on the individual's preference. The role also includes administrative responsibilities, such as supporting with triaging, liaising with professionals, and following up on identified risks to ensure appropriate actions and support are in place. Travel within Bradford, Airedale, Wharfedale, or Craven is required to meet with service members and offer support

Role Purpose
  • Bradford District and Craven Mind is looking for a dedicated person committed to delivering high-quality mental health care and support under the SM-ILE service. You will complete 1:1 support with adults facing moderate to severe mental health challenges. The support will be to offer personailised support, around mental health and wellbeing, social support, housing, welfare and benefits, bereavement, employment, debt management. The job requires traveling within Bradford and Craven to meet service members and understand their needs in order to facilitate the personalised support. A good understanding of local support services across the district is essential, as the role involves signposting and completing onward referrals for service members to relevant services for additional support
Role Outcomes
  • Complete 1- 1 support for adults facing moderate to severe mental health challenges face-to-face and or over the telephone
  • Provide support that meets the service member needs. This support will be around client well-being, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement.
  • Have a good understanding of support services across the district and signpost/complete onward referrals for service members to relevant services for additional support
  • Work in partnership with other services across Bradford District and Craven to ensure the best outcome for service members
  • Record information electronically and maintain up to date service member records
  • Effectively managing risk in a multi-agency way through timely completion of risk assessments and safety plans
  • Actively engage in supervision, team meetings and company events to contribute to a well-connected, informed and supportive team environment
  • Take responsibility for ensuring Safeguarding, Information Governance and Equality & Diversity requirements, and all other Company policies are complied with and seek to improve these where possible.
  • Work in alignment with the policies, aims, objectives and values of Bradford District and Craven Mind
  • Perform any other duties from time to time that may be reasonably required to ensure the smooth running of Bradford District and Craven Mind

Essential
  • Understanding of the key issues within mental health, and the potential challenges that people may face
  • Knowledge of other relevant health & social care organisations across the Bradford District & Craven
  • Knowledge of risk management and health and safety issues

  • Understanding of confidentiality and data protection
Experience:
Essential
  • Experienced and confident in managing challenging situations, and recognising and responding to safeguarding concerns (this can include voluntary experience)
  • Experience of working with children, young people or adults in a health, social care or any other relevant setting (this can include voluntary experience)
  • Experience of supporting people in a clear, meaningful and non-judgemental way either via telephone or in person (this can include voluntary experience)

  • Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives.
  • Excellent IT skills, including use of Microsoft Packages
