Job summary

University Hospitals of Derby and Burton NHS Foundation Trust is seeking a Pharmacy Technician for ward-based Medicines Management. The role covers reconciliation, patient counselling and ensuring timely, safe discharge prescriptions across multiple wards at Royal Derby Hospital.

Qualifications

• Registered as a Pharmacy Technician or due to qualify soon.
• Experience with medicines reconciliation and ward-based processes is desirable.
• Knowledge of pharmacy practice, law and ethics in a hospital setting.

Responsibilities

• Provide ward-based Medicine Management and Optimisation, including reconciliation and patient counselling.
• Ensure safety and timeliness of discharge prescriptions.
• Support MDT with pharmacists, doctors and nurses.
• Obtain accurate medication histories from multiple sources and reconcile with inpatient prescriptions.
• Communicate drug doses, indications, side effects and efficacy to patients and carers.
