Qualifications

• Provide medicines management on an inpatient ward including drug history and reconciliation.
• Check prescriptions dispensed by pharmacy staff for safety and accuracy in ward and dispensary.
• Be part of the ward MDT and pharmacy weekend/bank rota and respond to needs of the team.

Responsibilities

• Provide a medicines management service to an inpatient ward including drug history, and medicines reconciliation.
• Responsible for safe, efficient and effective checking of prescriptions dispensed by other members of the pharmacy team in the dispensary and ward based.
• Being part of the ward MDT and the wider pharmacy team. This includes being part of the pharmacy weekend and bank holiday rota.
