Dorset County Hospital NHS Foundation Trust is seeking a Clinical Pharmacy Assistant to support medicines management across wards and discharge pathways. You will complete a medicines management qualification and apply SOPs to assess patients' own medicines, ensure accurate dispensing, and coordinate at discharge with the Pharmacy Medicines Management Team.

You will work 37.5 hours per week, including weekends, Bank Holidays and late duties on a rotational basis, with training on pharmacy

Qualifications

• Must hold a medicines management qualification.
• Excellent communication skills and professional conduct.
• Ability to work as part of a team and show initiative.

Responsabilités

• Assist the Medicines Management Team at ward level to ensure smooth discharge and accurate assessment of patients' own medicines.
• Order medicines as needed to avoid missed doses and support discharge processes.
• Help with transfer of medicines between wards, labelling and dispensing for patients, and manage returns from wards.
