VAWAS Nurse Practitioner
Peterlee (County Durham)

A Good CQC rated GP Surgery in County Durham is looking to hire a Nurse Practitioner to carry out Home Visits for local patient homes and Care Homes. Patients will consist of vulnerable adults and the elderly. When carrying out the Home Visits, duties will include:

1. See & Treat
2. Clinical Assessment
3. History Taking
4. Minor Illness
5. Dementia
6. Long Term Conditions
7. Treatment Plans
8. Medication Reviews

The successful candidate will hold a Prescribing qualification. The practice is looking for someone to offer 34 hours over 4 days (including 1 weekend per month).
