Job summary

East Of England Community Health And Care NHS Trust is seeking a qualified nurse to lead delivery of high standards of care in patients' homes, assessing, planning, implementing and evaluating care to prevent hospital admissions and support early discharge. You will frequently lead discussions with the Urgent Community Response MDT.

The role is predominantly face-to-face nursing in patients' usual place of residence.

Qualifications

  • Registered general nurse with current NMC registration.
  • CPD evidence of continuing professional development.
  • Diploma or 1st level degree in Nursing or equivalent.
  • Experience at Band 5 in community setting.

Responsibilities

  • Assess, plan, implement and evaluate all aspects of patient care and develop treatment/care pathways.
  • Holistic assessment and individualised care pathway planning.
  • Utilise Assistive Technology where required.
  • Communicate complex information to patients and carers.
  • Adjust explanations for barriers to understanding; gain consent and concordance.
  • Establish and maintain therapeutic relationships with patients and carers.
  • Liaise with GPs, Social Services and other professionals to coordinate care.
  • Undertake risk assessments and act on them appropriately.
  • Participate in multi-disciplinary meetings and organise specialist services.
