Job summary

Kernow Health CIC in Cornwall is seeking an Advanced Practitioner to deliver urgent care assessment and management across the 111 Clinical Assessment Service, with face-to-face centres and home visiting settings. You will practice safely and independently within your scope of competence, and ensure timely follow-up or referrals as needed.

The role involves autonomous clinical decision-making, collaboration with GPs and the wider multidisciplinary team, and adherence to governance and quality

Responsibilities

• Autonomous clinical practice and safe, independent decision-making within scope of competence.
• Conduct comprehensive history taking, physical examination, and clinical assessment.
• Formulate and implement management plans and arrange safe follow-up or onward referral.
