Central London Community Healthcare NHS Trust in London is recruiting a Band 5 Community Nurse for Twilight & Night Service based in Brent. You will provide a mix of general and specific care to patients in their own homes, supporting independence and patient-centred care within a busy, multidisciplinary team.

The role requires a registered nurse with community experience, working across seven days with evening and night shifts.

Qualifications

• Registered nurse with current NMC registration.
• Experience in community or district nursing is preferred.

Responsabilités

• Provide general and specific care to patients in their own homes.
• Work under supervision within Twilight & Night Service.
• Promote independence and patient-centred care.
• Support community nursing team to meet KPIs and service standards.
