Job summary

Qualifications

• Registered General Nurse with active NMC registration.
• Demonstrate evidence-based practice and up-to-date clinical skills.
• Ability to travel across sites to deliver care.
• Driving license and access to a car are required.

Responsibilities

• Deliver responsive, high-quality community nursing to patients at home.
• Work with the MDT to support patients with complex needs.
• Promote self-care and health promotion activities.
• Support urgent response and virtual ward initiatives within the team.
