Job summary

West London NHS Trust is recruiting a Band 5 Community Staff Nurse to deliver care in patients' homes, helping prevent hospital admissions and support safe discharges. You will work within a multidisciplinary team, coordinating with GPs, LAS and Social Services to maintain patient safety and independence.

Responsibilities

• Assess, plan, implement and evaluate nursing care for patients in their own homes.
• Support admission avoidance and safe, timely discharge from hospital.
• Work as part of a multidisciplinary team and liaise with GPs, London Ambulance Service and Social Services.
• Deliver high-quality, evidence-based and person-centred care with good communication, organisation and time-management.
• Demonstrate flexibility by working across a 7-day service and showing initiative.
