Cumberland Council invites applications for a Trusted Assessor based in a hospital setting to work with Case Managers, Discharge Navigators and multidisciplinary partners on Discharge to Assess pathways. The role ensures safe, timely discharges to health and social care services, including community and building-based provision.
The position is 36 hours per week, temporary for six months, with a salary of £39,481 to £40,444 per annum and based at Allerdale House, Workington.
As a Trusted Assessor within a hospital-based service, you will work collaboratively with Case Managers, Discharge Navigators, and multidisciplinary partners across all Discharge to Assess pathways. The role focuses on supporting and facilitating safe, timely, and effective discharges from hospital into a range of health and social care services, including both community-based and building-based provision.
