Salary: £39,481 to £40,444 per annum

Hours: 37 hours per week

Location: Workington

Contract: Permanent

Work within a District or Locality team to assess social care needs and deliver person-centred support that promotes independence, wellbeing, choice, and control for people in Cumbria. Manage a caseload, coordinate outcomes, assess and manage risk, and work within financial boundaries. Develop strong partnerships with individuals, communities, and agencies to maximise access to local assets, preventative support, and services using a strengths-based approach. Operate autonomously within a multi-disciplinary environment, handling complex cases while contributing to continuous service improvement and positive outcomes for the people of Cumbria.
