Telford & Wrekin Council is recruiting a Triage Officer to join the Making Prevention Real pilot within Adult Social Care. This 12-month fixed-term contract is 37 hours per week and sits at the front door of the service.

You will gather information, coordinate referrals and support timely decisions with health partners to provide early intervention and improved access to services.

The role offers hybrid working, DBS check, and opportunities for development within a forward-thinking council.
