Job description

An exciting opportunity has arisen for experienced, enthusiastic and motivated Nurses or Paramedics to join our hard working team and develop new ways of working.

37.5 hours across 7 days a week.

We are looking for Clinical Triage Clinicians to join the Dudley Clinical Hub (Single Point of Access) offering triage, clinical advice and signposting. The postholder will be responsible for the appropriate clinical streaming of patients attending the Emergency Department, being accountable for effective streaming of patients into community services.

The post holder will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multidisciplinary team in delivering services within an Emergency Department setting in a widely diverse locality.

The successful applicants will join an integrated, collaboratively focused team, employing a proactive clinical practice and knowledge to provide clinical triage / clinical assessment skills to support urgent care for patients across our health economy.

The service operates between the hours of 8am - 9pm, 7 days per week, and 365 days per year. The successful applicants will be expected to work flexibly throughout this period. The service offers extensive support and knowledge to enable patients to be managed away from the acute setting and to implement the right level of care support to achieve this.

Main duties of the job

We need applicants to be flexible, approachable and have a sound clinical evidence based background and knowledge. They will need to be friendly but assertive and be confident in their own decision making; being able to challenge when necessary.

To provide clinical support, assistance, and professional expertise to all healthcare professionals in support of avoiding inappropriate hospital admissions by referring onto the most appropriate community service for the patient's needs, including the Urgent Community Response Team.

The primary objectives are to prevent unnecessary admission to acute care and to facilitate collaborative MDT management for patients closer to home. The role involves triaging referrals from GP's, West Midlands Ambulance Service, Community Nurses and all other Health Professionals. The triage clinician will be expected to visit patients in their own home supported by the Medical Lead and Senior Practitioners if required. This will enable the clinician to receive further training to ensure clinical skills and competencies are maintained.

We are looking for enthusiastic and innovative individuals who possess excellent communication, customer care and IT skills. The candidates will have to be able to prioritise their own workload without supervision, have an excellent telephone manner and be able to work well on their own initiative as well as part of a team.
