Job summary

The East Kent Colleges Group is seeking a compassionate Transition Mentor to support vulnerable 16-19 learners through pre-enrolment to the first 100 days of study. You will deliver targeted transition, mentoring, wellbeing and attendance support, building trusting relationships with learners, carers, schools and multi-agency partners.

You will apply a learner-centred, trauma-informed approach and contribute to safeguarding, inclusion and learner wellbeing across the journey, including accurate

Qualifications

• Experience supporting young people in education, youth work or similar setting.
• Experience supporting learners with SEMH needs, care experience or attendance challenges.
• Excellent communication and listening skills, with the ability to build trust.
• Learner-centred and trauma-informed approach to supporting young people.
• Strong organisational skills and ability to maintain accurate, confidential records.
• Experience working with families, external agencies and other professionals.
• Commitment to inclusion, equality, safeguarding and learner wellbeing.

Responsibilities

• Deliver targeted transition, mentoring, wellbeing, and attendance support to vulnerable and disadvantaged 16–19 learners.
• Focus on pre-entry engagement, structured induction, early attendance mentoring, and sustained relational support during transition into vocational/technical education.
• Work with learners, carers, schools and multi-agency partners to deliver personalised support from pre-enrolment through the first 100 days of study.
• Maintain accurate, confidential records of support and progress.
• Contribute to safeguarding and wellbeing initiatives across the learner journey.
