Job summary

Leeds Community Healthcare NHS Trust invites applications for a practitioner to join the new Proactive Support Service. We welcome qualified Registered Nurses (RMN/RNLD) and Social Workers with experience supporting children and young people with complex needs, trauma‑informed and relational approaches to care.

You will work across agencies, providing assessment, direct intervention and system‑wide support to reduce crisis and placement instability.

Qualifications

• Registered RMN or RNLD with registration.
• Experience in trauma‑informed and relational approaches.
• Experience with complex needs and CNOD pathway is desirable.

Responsibilities

• Provide assessment, consultation, direct intervention and system-wide support.
• Work closely with families, carers, residential providers, social care, education and health colleagues.
• Help networks around the child respond to distress through trauma‑informed practice.
