Qualifications

  • Qualified Social Worker with 3 years post-qualification experience.
  • Extensive experience with a wide range of young people and adults including disabled, Autistic and Mental Health conditions.
  • Strong legal literacy related to Adults and Children’s Social Care, including SEND legislation.
  • Experience collaborating across Adult and Children’s Social Care and wider partnerships.
  • Registered with Social Work England.
  • Full UK Driver’s Licence and access to a car for business needs.

  • Oversee learning development opportunities and student placements in conjunction with the Practice Development Lead.

About The Team

We are looking for a passionate Transitions Senior Practitioner to join our Specialist Adult Social Care services, which are based in Keynsham. This role is a specific role within the transitions team, working with young people (16 to 25) with the appearance of social care needs transitioning into adult care.

As a Senior Practitioner, you will be responsible for providing expert case consultation, practice guidance, and professional support to colleagues, helping to ensure that decisions are robust, legally literate, and aligned with our practice standards and frameworks. You will support the development of high-quality practice across the team through coaching, mentoring, and reflective discussions.

You will join a friendly and extremely supportive team of Social Work professionals, giving you the opportunity to work within a multi‑disciplinary role liaising with other partners and agencies, including Avon and Wiltshire NHS Partnership Adult Mental Health Teams, CAMHs, SEND Practitioners, Children’s Services, Psychologists, Occupational Therapists, Speech and Language Therapists and education professionals.

About The Role
  • Where it is appropriate, to be responsible for a small number of complex, high‑risk cases, reflective of your experience and expertise.
  • Where necessary to co‑work cases alongside other team members where complexity or legal requirements require additional support.
  • Provide ongoing case, practice advice and practice development discussions with team members as required to ensure high quality and legally literate decisions are made in line with ASC Practice Framework.
  • To have oversight of team safeguarding activity, including monitoring referrals, allocation and meetings with safeguarding chairs to agree next steps.
  • Where appropriate to chair MARM or MDT meetings under the direction of the service lead.
  • Being the senior practice lead by representing the service in the practice development and disseminating learning and supporting training needs of the deliver.
  • Lead and contribute to a range of service‑related projects as they arise, including the development of new initiatives through representation on working/multi‑agency groups.
  • Undertake audits of practice in line with the audit framework supporting staff with evidence based and reflective practice.
  • To take a lead and have oversight of learning development opportunities and student placements in the team in conjunction with the department’s Practice Development Lead this includes maintaining PEPS qualification where appropriate.
About You
  • Be a qualified Social Worker (Social work degree, Diploma in Social Work, CQSW or equivalent) with 3 years post qualification experience
  • Have extensive experience working with wider range of young people and adults including people with physical disability, Autism, Mental Health Conditions or Learning Disabilities.
  • Able to demonstrate and evidence a high level of legal literacy related to both Adults and Children’s Social Care, including legislation relating to Young People with SEND.
  • Experience of working collaboratively across Adult Social Care, Children’s Social Care and wider partnerships.
  • Registered with Social Work England
  • Have a Full UK Driver’s License and access to a car due to business need
