Job summary

Bath and North East Somerset Council is seeking a Transitions Senior Practitioner for the Specialist Adult Social Care team based in Keynsham. You will oversee complex cases and co-work as needed, ensuring high-quality, legally literate decisions in line with the ASC framework.

You will lead safeguarding activities, chair meetings, and contribute to staff development and learning opportunities within the team. A strong emphasis on collaboration across Adult and Children’s Social Care is required.

Qualifications

• Qualified Social Worker with 3+ years post-qualification experience.
• Experience with young people (16–25) and adults including disability, autism, MH or LD.
• Strong legal literacy for Adults and Children's Social Care, incl. SEND.
• Registered with Social Work England and valid UK driving licence.

Responsibilities

• Manage a small number of complex, high-risk cases as appropriate.
• Co-work cases with other team members when needed.
• Provide ongoing case and practice advice and development discussions.
• Oversee safeguarding activity: referrals, allocations, meetings with safeguarding chairs.
• Chair MARM or MDT meetings when directed by service lead.
• Lead learning development and student placements with the Practice Development Lead.
• Contribute to service-related projects and audits of practice.

About the Team

We are looking for a passionate Transitions Senior Practitioner to join our Specialist Adult Social Care services, which are based in Keynsham. This role is a specific role within the transitions team, working with young people (16 to 25) with the appearance of social care needs transitioning into adult care.

About the Role

As a Transitions Senior Practitioner, you will carry out responsibilities including:

• Where it is appropriate, to be responsible for a small number of complex, high-risk cases, reflective of your experience and expertise.
• Where necessary to co-work cases alongside other team members where complexity or legal requirements require additional support.
• Provide ongoing case, practice advice and practice development discussions with team members as required to ensure high quality and legally literate decisions are made in line with ASC Practice Framework.
• To have oversight of team safeguarding activity, including monitoring referrals, allocation and meetings with safeguarding chairs to agree next steps.
• Where appropriate to chair MARM or MDT meetings under the direction of the service lead.
• Being the senior practice lead by representing the service in the practice development and disseminating learning and supporting training needs of the deliver.
• Lead and contribute to a range of service-related projects as they arise, including the development of new initiatives through representation on working/multi-agency groups.
• Undertake audits of practice in line with the audit framework supporting staff with evidence based and reflective practice.
• To take a lead and have oversight of learning development opportunities and student placements in the team in conjunction with the department’s Practice Development Lead – this includes maintaining PEPS qualification where appropriate.

About You

• Be a qualified Social Worker (Social work degree, Diploma in Social Work, CQSW or equivalent) with 3 years post qualification experience
• Have extensive experience working with wider range of young people and adults including people with physical disability, Autism, Mental Health Conditions or Learning Disabilities.
• Able to demonstrate and evidence a high level of legal literacy related to both Adults and Children’s Social Care, including legislation relating to Young People with SEND.
• Experience of working collaboratively across Adult Social Care, Children’s Social Care and wider partnerships.
• Registered with Social Work England
• Have a Full UK Driver’s License and access to a car due to business need
