Job summary

United Lincolnshire Teaching Hospitals NHS Trust is seeking a Macmillan Cancer Care Coordinator in Transitional Oncology. The role supports CNS, MDTs and divisions to provide holistic, patient-centred care for those with palliative cancer needs.

The post is trust-wide and involves coordinating across services to ensure smooth care pathways. The successful candidate will complete Holistic Needs Assessments and link patients to district nursing and benefits support as appropriate, primarily

Qualifications

• NVQ Level 3 or equivalent plus diploma-level training or similar
• Previous acute hospital experience
• Experience using IT systems and working in multi-disciplinary teams
• Evidence of working within the cancer pathway

Responsibilities

• Support CNS and extended cancer MDT as directed
• Provide holistic needs assessments and patient support
• Signpost to district nursing and benefits agencies as needed
• Coordinate care across Trust, tertiary providers and primary care
• Assist with transition from curative to palliative oncology care
