Job summary

Macmillan Cancer Care Coordinator in Transitional Oncology is a Band 4 NHS post with United Lincolnshire Hospitals NHS Trust, located across ULHT sites. The role supports Clinical Nurse Specialists within the Transitional Oncology Team to provide high-quality, person-centred care for patients transitioning from curative to palliative care.

Qualifications

• NVQ Level 3 or equivalent level of demonstrable experience, plus additional short courses or training to diploma level or equivalent
• Previous acute hospital experience
• Experience of multi professional working
• Experience using IT systems
• Able to demonstrate "evidence" of working within the multi-disciplinary team
• Previous cancer pathway experience

Responsibilities

• The post holder will be an extended member of the cancer MDT, to support as needed, ensuring effective communication across the Trust, Tertiary providers, Primary Care and statutory and voluntary agencies.
• Patients and their loved ones are supported holistically with their physical, emotional, spiritual, psychological and financial needs throughout their palliative cancer journey. This can be supporting their transition from curative to palliative oncology treatment, cancer progression or best supportive care.
• You will be a point of contact for patients during their cancer journey, or their relatives, to answer questions and deal with problems that arise, under the guidance of the CNS, linking in or signposting to services such as district nursing and benefits agency as appropriate
• The role involves completing Holistic Needs Assessments, patient support and completing tasks as required by the CNS team.
