Mid Cheshire Hospitals NHS Foundation Trust is seeking a Band 4 Therapy Assistant to join the IPOCH Home First Team. The role covers the Emergency Department, admission avoidance pathways, inpatient wards and community settings to support care in the most appropriate setting and prevent unnecessary hospital admissions.

You will work with a multidisciplinary team, contribute to discharge planning, and undertake ongoing assessments while utilising electronic records.

Responsabilidades

• Make therapeutic contact with newly referred patients as identified by qualified therapists following admission and triage.
• Independently complete specified holistic assessments, interventions and rehabilitation programmes with or without clinician assistance for community-based patients.
• Follow departmental protocols and guidelines referring to the qualified clinician for advice.
• Hold a caseload as delegated by the qualified clinician, report back at agreed timescales, and support Band 3 Therapy Assistants as required.
• Work with individuals in settings such as home or 24-hour care facilities, ensuring patient safety and dignity.
• Modify interventions and programmes within defined parameters and within scope of practice.
• Complete initial assessments and commission basic equipment and adaptations as appropriate to the service user; review risk assessments regularly.
• Responsible for ordering and stock control of materials and equipment for the unit.
• Instruct individuals, carers and professionals in safe use and maintenance of equipment.
• Refer patients to appropriate services when complex health needs are identified.
• Discharge patients within workload after consultation with the team; attend multidisciplinary meetings and contribute to clinical decisions.
