Job summary

The YOU Trust is seeking a Community Health Worker to support individuals across Weymouth, Portland and the West of Dorset. The role involves 25 hours per week on a temporary 9‑month basis, delivering one‑to‑one support in clients' homes and in the community to help maintain independence and wellbeing.

You will manage caseloads, assist with housing and benefits applications, budgeting and access to local services, while travelling across Dorset to reach clients. A driving license is required.

Qualifications

• Awareness of risks to homelessness and impact on health and wellbeing.
• Broad knowledge of housing-related support, housing law, debt and welfare benefits.
• Experience with diverse client groups.

Responsibilities

• Manage own caseload, provide one-to-one support in home or community.
• Support clients via drop-ins in local areas.
• Navigate housing applications, benefits applications and budgeting.
• Encourage access to community resources and services.
• Partner with agencies and advocate for clients.
• Attend regular local office meetings in Dorchester.
• Travel across Dorset to meet client needs.
• Build positive community relationships and provide guidance.
• Work in partnership with clients as an equal.
