Hertfordshire County Council is seeking a Hospital Team Manager in Stevenage to lead a fast-paced Transfer of Care Hub. The role oversees a seven-day discharge service (08:00–18:00, weekends included), with one full weekend off every eight weeks and time off in lieu where applicable.

The post holder will provide operational leadership, ensure person-centred practice, and support performance management across hospital sites and community settings.

Qualifikationen

• Registered Social Worker status required.
• Experience leading teams in high-demand environments.
• Strong leadership with ability to manage priorities.

• Lead and oversee day-to-day practice within TOCH team.
• Manage team performance for timely hospital discharges.
• Inspire and develop a team of Social Workers and Community Care Officers.
• Build relationships with hospitals and community partners.
• Oversee resources, budgets, quality assurance and service improvement.
• Lead safeguarding enquiries and complex concerns.
