Job summary

Herts Legal Service - Hertfordshire County Council in Hemel Hempstead is seeking a Post Hospital Review Team Manager to lead our Integrated Discharge Service, coordinating with NHS colleagues to support discharge pathways and promote independence.

You will provide strong operational leadership, manage performance, and develop a busy team of Social Workers, Occupational Therapists and Nurses within a seven‑day discharge service, with weekend rotation and a driving requirement.

About The Team

Join our Integrated Discharge Service, a fast-paced, multi-disciplinary service delivering a coordinated and integrated approach supporting people on reablement and Discharge to Assess pathways after a hospital discharge.

The Post Hospital Review Team plays a critical role within the wider health and social care system, working in close partnership with NHS colleagues and other stakeholders to support individuals after a hospital admission with a focus on Home First and promoting independence.

About The Role

As Team Manager, you will provide strong operational leadership within a dynamic environment, ensuring high-quality, person-centred practice alongside robust performance management to support system flow and improved outcomes for residents. You will contribute to the effective management of performance,provide professional oversight, and support service development within a busy, fast-paced and evolving service.

The successful applicant will demonstrate sound knowledge of safeguarding practice, experience working with people with dementia and other long term health conditions, and a strong understanding of the personalisation agenda and Individual Budgets. You will ensure individuals and their families remain central to all decision‑making, delivering strengths‑based support within the legislative framework.

We are a busy service covering the large and diverse geographical area of Watford, Three Rivers and Hertsmere localities and are seeking a motivated and experienced leader who can bring energy, innovation and a clear focus on performance, quality and continuous improvement.

The Post Hospital Review Team operates a seven‑day hospital discharge service working one full weekend every eight weeks, with time off in lieu provided.

Key Responsibilities Include

• Leading and overseeing day‑to‑day practice within the team, ensuring compliance with statutory duties, policies and procedures, and embedding a strong performance management culture.
• Managing team performance to ensure the timely and effective completion of discharge to assess pathways, reviews and assessments, supporting system pressures
• Inspiring, developing and managing a team of Social Workers and Community Care Officers to deliver innovative, responsive and person‑centred care.
• Building and maintaining strong working relationships with colleagues in the hospital setting, community services and wider partners to support integrated working.
• Ensuring effective management of resources and budgets, alongside quality assurance and service improvement activity.
• Acting as the lead manager for complex and high‑profile safeguarding enquiries and serious concerns.

Essential

About you

• Registered Social Worker, Occupational Therapist or Nurse
• Full driving licence
• Significant experience of managing teams within a busy, high‑demand environment
• Strong leadership skills with a proven ability to deliver performance and manage competing priorities
