ABOUT THE ROLE

Your focus as Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other responsibilities will include:

  1. Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  2. Assisting in the smooth transition of discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
  3. Monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met with records amended accordingly.
  4. Practicing safe systems of work across tasks and moving and handling of residents by assessing risks and ensuring safety for residents, visitors, and staff.
ABOUT YOU

Our ideal candidate must:

  • Have an NVQ Level 3 or equivalent in care for care homes.
  • Have two years’ experience working in a senior position with residents.
  • Be able to communicate effectively both verbally and in writing.
  • Demonstrate leadership skills and act responsibly in the absence of the manager.
