Job summary

Radis Community Care is seeking an Extra Care Team Leader to join our team at Diamond Batch in Weston-super-Mare. You will split duties 50/50 between care delivery and team leadership, guiding Support Workers and ensuring high-quality, person-centred support for residents living in their own homes within the extra care complex.

You will be the first point of contact for the care team, conduct supervision and meetings, coordinate schedules, and help with daily living tasks as needed.

Qualifications

• One year minimum in a Team Leader/Senior role.
• NVQ Level 2 in Health and Social Care required, with willingness to progress to Level 3.
• Strong written and verbal communication skills.
• Proactive attitude with ability to adapt to changing needs.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well‑being.

You will be the first point of contact for the care and support provided, with responsibilities including:

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.

Skills and Qualifications
