Job summary

Radis Community Care is seeking an Extra Care Team Leader to lead our team at The Rise, George Lane, Plympton, Plymouth, Devon. You will oversee care delivery in an extra care housing complex, balancing supervision and hands-on support to help residents maintain independence.

The role combines people leadership with direct care tasks, including mentoring Support Workers, coordinating schedules, and ensuring high standards of person-centred care.

Qualifications

• Minimum of one year in a Team Leader / Senior role.
• NVQ Level 2 in Health and Social Care required, with a willingness to work toward Level 3.
• Strong written and verbal communication skills.
• A proactive attitude and adaptability to changing needs.

Responsibilities

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well-being.

You will be the first point of contact for the care and support provided, with responsibilities including:

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.
