Job summary

Swanton Care is opening a new Supported Living Service in Colne and seeks a dedicated Team Leader to establish and develop the service. You will lead a team to deliver high-quality, person-centred support in people’s own homes and community settings.

You will oversee daily operations, promote independence, and ensure robust care plans and DBS checks. The role requires full-time hours and weekend shifts, with 24/7 support provided by the service.

• Willingness to pursue relevant qualification if not already held.

About The Role

We are excited to be opening a new Supported Living Service in Colne and are looking for a dedicated Team Leader to join our core team. This is a fantastic opportunity to play a key role in establishing and developing a new service that supports people to live as independently as possible within their own homes and local community.

Hours: Full-time, 40 hours per week

About You

Are you enthusiastic, compassionate, and committed to supporting people in a way that respects their dignity and promotes independence?

You will be adaptable and flexible, with excellent communication skills and the ability to lead and support a team while working collaboratively to deliver outstanding person-centred care.

It is desirable that you have:

• Previous experience supporting people with learning disabilities and behaviours of concern.
• Experience in a senior support or leadership role within health and social care.
• An NVQ Level 3 in Health & Social Care, or willingness to work towards achieving this qualification.

The Role

As Team Leader, you will play a vital role in supporting the registered manager and leading a team to deliver high-quality, person-centred support. You will help create a positive and empowering environment where the people we support can achieve their goals, make informed choices, and live fulfilling lives.

Support is provided 24 hours a day, 7 days a week.

Key responsibilities include:

• Leading, motivating, and supporting a team of support workers to deliver excellent care and support.
• Providing practical assistance and emotional support to empower the people we support to achieve their goals and aspirations.
• Promoting the health, wellbeing, physical, emotional, cultural, and spiritual needs of the people we support in an inclusive way.
• Supporting individuals to access social, educational, vocational, and leisure activities, including visiting family and friends, shopping, community activities, and recreational pursuits.
• Assisting with daily living tasks, including household duties, budgeting, shopping, and elements of personal care where required.
• Ensuring support plans, risk assessments, and records are completed accurately and maintained to a high standard.
• Supporting the successful development and growth of this new service in Colne, helping to build a positive team culture and excellent outcomes for the people we support.

About The Service

We provide support to adults with a range of disabilities including learning disabilities, autism, physical disabilities, mental health problems and sensory impairment in the Northwest of England. We provide local, personal and responsive services. We do this by remaining small and staying close to the people we support and the staff we employ.
