Main duties of the job

As a Team Leader at Avery Healthcare, your focus will be to support the home management team in delivering high-quality, person-centred care. You will implement, monitor, and evaluate care plans, assist with resident discharges, and ensure the well-being of each resident. You will also lead staff and act responsibly in the absence of the manager, practicing safe systems of work and maintaining a safe, caring, and inclusive environment.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Qualifications
  • To succeed in this role, you must have an NVQ 3 or equivalent in care for care homes, and at least two years' experience working in a senior position with the resident group.
