Package Description

Shift Details: Two positions available: one for 33 hours and another for 44 hours.

About The Role

Your focus as Team Leader will be to support the home management team in delivering high-quality care by creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other Responsibilities Will Include

  • Leading in the person-centred care planning process, including implementing, monitoring, evaluating, and auditing care plans, developing action plans, and ensuring their completion.
  • Assisting in the smooth discharge process, including documentation, medication management, and keeping residents' next of kin informed where appropriate.
  • Monitoring residents' well-being, recognizing changes impacting care, updating care plans accordingly, and ensuring records reflect any variations.
  • Practicing safe systems of work, assessing risks, and ensuring safety during resident handling and other tasks.

About You

Our Ideal Candidate Must

  • Hold an NVQ Level 3 or equivalent in care for care homes.
  • Have at least two years’ experience in a senior role with residents.
  • Be an effective communicator, both verbally and in writing.
  • Demonstrate leadership abilities and responsibility in the absence of the manager.
