Job summary

Partnership of Care in South Wales is seeking an experienced Team Leader in Supported Living to lead and support a dedicated team on shift. You will oversee medication administration, safety plans and risk assessments, and promote independence through life skills coaching.

The role offers permanent or part-time contracts with a focus on person-centred care, ongoing training, and genuine career progression opportunities. A full driving licence is required to fulfil duties.

Key Responsibilities:

• Working as a team player while leading, guiding and supervising while on Shift.
• Administering medication, including ensuring medication counts are correct and checking other competencies.
• Designing and implementing with your line manager support plans and risk assessments, ensuring the team with you always work to them.
• Support those who need it with food preparation and other life skills to promote independence.
• Assist the people we support to develop and maintain relationships with family and friends.
