Job summary

Avery Healthcare Group is seeking a Team Leader for Acer Court Care Home in Nuthall, Nottinghamshire. You will support the home management team to deliver high-quality, person-centred care and lead a team in line with our values.

Responsibilities include leading monthly care plan evaluations, managing discharges safely, monitoring resident wellbeing, maintaining safe working practices, and developing staff in the absence of the manager.

Qualifications

  • Must have NVQ 3 or equivalent in care for care homes.
  • Two years' experience in a senior position with the resident group.
  • Ability to communicate effectively both verbally and in writing.

Responsibilities

  • Lead person-centred care planning with monthly evaluations and audits, developing action plans.
  • Assist in smooth discharge transitions, including documentation, medication, and informing next of kin.
  • Monitor resident wellbeing, update records for changing needs, and ensure care plans reflect variations.
  • Practice safe systems of work, including moving and handling of residents, with risk assessments.

  • Lead staff and act in the absence of the home manager.

Job description

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive , and inclusive environment for residents, visitors, and staff. Other responsibilities will include:

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
