Job summary

Real Life Options Group is seeking an experienced Team Coordinator in West Lothian to lead a team delivering person-centred care across Supported Living, Outreach and Day Centre services. You will oversee around 25 people, manage rotas, risk assessments and ensure high standards of safety and governance.

The role requires a SVQ3 in Health & Social Care, 3+ years in a similar setting, strong leadership and driving. A flexible, values-led approach is essential.

Qualifications

• SVQ Level 3 (or equivalent) in Health & Social Care – essential.
• Minimum of 3 years’ experience in a similar social care role – essential.
• Proven supervisory/leadership experience.
• Excellent communication and interpersonal skills.
• Driver with access to a vehicle – essential.

Responsibilities

• Lead and support staff to deliver person-centred care.
• Manage rotas, risk assessments, audits and health & safety compliance.
• Supervise, mentor and develop Support Workers with appraisals.
• Embed the Person-Centred Support model across the service.
• Liaise with social workers, families and external agencies.
• Deputise for the Service Manager when required; participate in on-call duties.
