We are pleased to offer an exciting opportunity to join our Palliative Care Specialist Team in one of the country’s largest acute NHS Trusts as the team leader.
The post will be based within the existing Specialist Palliative Care Team, offering ongoing support and working alongside staff to ensure that people with specialist palliative care needs receive the care they require to achieve the best quality of life possible. The post will be based at The Royal London in Whitechapel; there may be some rotation and cross-site working.
This post requires a highly skilled individual with extensive relevant palliative care experience to support the delivery of clinically effective, efficient, and high-quality care to people with complex specialist palliative care needs, ensuring that this care is evidence-based and reflects current best practices.
Using specialist palliative skills and knowledge, you will manage a caseload of patients and their families/carers with complex needs related to living with a life-challenging illness.
This role involves contributing to a networked service to facilitate access to assessment, advance care planning, and treatment for a specific cohort of patients and their families with complex palliative care needs. You will work in partnership with generic ward staff and specialist community teams, especially for those being rapidly discharged home for terminal care.
You will act as a specialist resource across the whole system, influencing patient care through a team approach by providing specialist education and training to multi-professional staff/students, statutory and voluntary agencies. Additionally, you will undertake research, audits, and service development within palliative and end-of-life care.
This post will also proactively support a move towards 7-day working within the existing CNS team, with main hours covering weekends and bank holidays.
