Job summary

Milestones Trust in Swindon is seeking a compassionate Support Worker to assist people living in supported living and outreach services in Stratton St Margaret. You will promote independence, dignity and wellbeing while enabling people to live in their own homes and communities.

The role requires flexibility to cover evenings, weekends and bank holidays, and involves personal care, medication administration, and routine documentation.

Responsibilities

  • Support people with daily living tasks including cooking, cleaning, shopping, budgeting and appointments.
  • Provide personal care where required and support individuals to maintain health and wellbeing.
  • Administer medication in line with training and policies.

  • Follow support plans, risk assessments and positive behaviour strategies.
  • Maintain accurate records and complete necessary documentation.
  • Identify and report safeguarding concerns promptly.
  • Collaborate with colleagues, families and professionals to achieve positive outcomes.

Job description

We are looking for a compassionate and enthusiastic Support Worker to join our team in the Stratton St Margaret area of Swindon. Working across supported living and outreach services, you will provide person-centred support, promoting independence, choice, dignity, and wellbeing while enabling people to live fulfilling lives in their own homes and communities.

Key Responsibilities
  • Support people with daily living tasks, including cooking, cleaning, shopping, budgeting, and attending appointments.
  • Provide personal care where required and support individuals to maintain their health and wellbeing.
  • Administer medication in line with training and organisational policies.
  • Encourage independence, choice, and community participation.
  • Follow support plans, risk assessments, and positive behaviour support strategies.
  • Maintain accurate records and complete necessary documentation.
  • Identify and report safeguarding concerns promptly.
  • Work collaboratively with colleagues, families, and professionals to achieve positive outcomes.
Essential Requirements

Desirable
  • Experience supporting people with learning disabilities, autism, mental health needs, or physical disabilities.
  • Health and Social Care qualification (Level 2 or above).
  • Full UK driving licence.
